Older hen problem

My four year old Sussex has developed a bent and twisted foot and she appears to be blind in both eyes. She is still feeding but her movements are difficult and she can't make it up the steps to the eggbox. She looks very unhappy and puffed up. The other hens are fine and are laying as normal.
I have isolated her from the others and would value some advice.
